New DOT Chain of Custody Forms

Please be advised that if you have not yet received the new DOT chain of custody forms that must be used effective July 1st, 2018, you should shortly. Once they arrive with this sheet (see below, click to view large), please dispose of the existing DOT chain of custody forms so that future tests have the correct form. If an old form is used after June 30th, 2018, a referendum will be sent to the collection site that used the old one to identify the error, but the test will still be processed. It will, however, delay the process of getting the test results. If you have questions, please call me at 701-516-3850.
